The first 72 hours following a hospital stay are a critical and vulnerable time for the individual.  This period often involves changes to daily habits, required medications, meal regimens, rehab exercises and more.

Our specially trained Care Workers provide transport home from hospital, providing essential support during an often stressful time. Once home, Care Workers provide monitoring, reminders and emotional support to help the individual recover successfully.
